Members of the LGBTQIA2s+ community demonstrate remarkable resilience, yet many continue to face unique challenges that can affect both mental health and substance use outcomes. Research has consistently shown that, compared to the general population, LGBTQIA2s+ individuals experience higher rates of:

For many LGBTQIA2s+ individuals (which we’ll collectively refer to in this article as LGBTQ), stress often begins early in life. Experiences such as family rejection, harassment, housing instability, workplace discrimination, or fear of being misunderstood by healthcare providers can contribute to chronic emotional distress. Over time, some individuals may turn to alcohol or drugs as a way to cope with overwhelming emotions, trauma, or social pressures. Studies continue to show that minority stress—the cumulative impact of prejudice and discrimination—plays a significant role in mental health and substance use disparities among LGBTQ populations.

Additionally, mental health and substance use concerns frequently occur together. Someone struggling with depression, anxiety, or trauma may also develop unhealthy patterns of alcohol or drug use. Likewise, substance misuse worsens existing mental health symptoms. Effective treatment recognizes this connection and addresses the whole person rather than focusing on a single diagnosis. 

 

Why Is It Important to Demand Affirming Healthcare?

LGBTQ individuals deserve healthcare environments where they’re treated with dignity, respect, and understanding. An affirming provider recognizes the impact that discrimination, social stigma, and identity-related stress can have on mental health and recovery.

Healthcare should be a collaborative process in which all patients feel heard and respected. You or a loved one has a right to feel empowered to ask questions when seeking care, including, but not limited to: 

  • Does the provider ask for and consistently use a patient’s chosen name and pronouns?
  • Are intake forms and medical records inclusive of diverse gender identities, sexual orientations, and family structures?
  • Does the provider have experience treating LGBTQ patients with concerns similar to mine?
  • How does the provider ensure patient privacy and confidentiality regarding sexual orientation and gender identity information?
  • Does the provider discuss treatment options without making assumptions about relationships, sexual behaviors, or family plans?
  • How does the provider address discrimination, bias, or concerns if a patient experiences them within the healthcare setting?
  • Does the provider consider the impact of social determinants of health, such as housing, employment, and community support, on patient well-being?
  • How are staff members trained to maintain cultural humility and stay informed about evolving LGBTQ healthcare best practices?
  • Does the practice visibly communicate its commitment to inclusion through policies, educational materials, or community engagement?
  • Are referrals available to LGBTQ-affirming specialists, mental health professionals, and community resources?
  • Are support services available for identity-related stress, minority stress, or experiences of stigma and discrimination?

These and other questions can help patients evaluate not only clinical competence but also whether the care environment is welcoming, respectful, and responsive to their individual needs.

Advocating for affirming care improves treatment engagement, strengthens therapeutic relationships, and helps people remain connected to recovery support. When you feel safe discussing your experiences openly, providers better understand your needs and develop more effective treatment plans.
